TrinityCore Custom quest bug

Were you using custom items in your quest? It seems like it happens randomly (most likely it's not random though, just an unknown reason), but only happens with custom items. It's happened to me before too.

I wouldn't say an unknown reason, since there is a reason for what is happening. After doing some testing on this for 6+ hours or so with my project team, this seems to happen 4% of the time which is still very low percentage. Still don't think anything custom would cause this. I still have a good feeling is might be related to syncing of the database to server.
