Updating Script/DB

I will be working on updating and old core/db to current from trinitycore. The one I'm using is 3-4 years old and is in need of some updating. I'm not worried about losing any of the custom C++ files. My main concern is the DB. Is it possible to update to the current core/db without losing the accounts, characters & custom things within the DB?

Yes, it is possible to update without losing anything. Back up your database and then update it. Though, if it is that old it probably wouldn't really be worth updating since there's been a ton of changes during that timeline to the core and database.

You say it's not worth updating? There's been a ton of updates including exploit fixes and the countless bug fixes, why would it not be worth it?
